SELF SCREENING POLICY FOR CYP FACILITIES AND PROGRAMS CYP requires parents to self-screen their child(ren) / teen(s) prior to attending the program daily using our CYP Enhanced Health Screening Questionnaire. CYPs will post the Enhanced Health Screening Questionnaire to our front doors. Any child/teen that enters the School Age Care Program, Child Development Center and the Youth Center or arrives at a Youth Sports & Fitness activity will be presumed to have completed a self-screening by their parent. As always, CYPs must follow regular exclusion criteria in the event that a child/teen displays illness symptoms while participating in the program. If this occurs, parents are required to pick-up their child/teen as soon as possible.
